The Underworld is the realm of Grenth. Many years ago, Dhuum, the cruel and unjust god of death who neglected his duties and enslaved the dead was deposed here.
When the body dies, the spirit will be released into the Rift. These spirits will find their way to the Underworld, normally via the River of Souls. Here the spirits are led to their final resting place, depending on numerous factors, such as how they performed in life, how they died etc.
More recently, this realm has become infected with the memory of the fallen god of death. Mysterious forces have been intercepting the River in the Realm of Torment, before it even reaches the Underworld. Demons have appeared across the realm, terrorizing the dead. In desperation, Grenth has opened up his realm to Ascendants from the mortal world to help cleanse his realm of evil.

To get to The Underworld, all players of the party must be Ascended, Weh no Su or must have completed the quest Hunted! (Elonian characters only). Players must then either use a Passage Scroll to the Underworld, or pay 1
to the Voice of Grenth at a Statue of Grenth while the world has the Favor of the Gods in any of the following locations:
- Chantry of Secrets
- Temple of the Ages
- Zin Ku Corridor
- South east corner of Lornar's Pass.

- All quests in the area will cause additional monsters to spawn.
- Henchmen cannot enter this area, but Heroes can.
- This area is among the most difficult in the game.
- All quests can be repeated upon re-entering the Realm.
- The Underworld is home to a unique pet, the Black Widow. This creature will spawn near a quest reward chest as level 20 and with a random evolution upon the completion of one of the following quests:
- Completing all quests in the Underworld at once will unlock a statue in the Hall of Monuments and allow each party member to open the Underworld Chest once.
- If all party members — or some of the allies — die, the party will get thrown out of the Underworld.
- The most common valuable drop in the Underworld is the Glob of Ectoplasm.
- There is a conversation that starts if you bring Gwen into the Underworld between her and her mother.
- During the Halloween 2009 event the Skeleton of Dhuum were introduced in a quest series. Later it was confirmed to be targeting the excessive farming of the Underworld, both solo and Speed Clear. quote Additional Skeletons spawn during several of the Underworld's quest series, such as with Unwanted Guests.
